Community Care Service Voucher for the Elderly

In addition to providing holistic and community-based health services for the elderlies, C & M A Tsui Lok Good Neighbours Centre for the Elderly also strives to become a (service) support centre for them as they manage their own health, and to care for their physical, emotional, and spiritual needs, encouraging them to live rich and productive lives - something that they will find enjoyable.

 

The centre has been a recognized service provider under the Second Phase of the “Pilot Scheme on Community Care Service Voucher for the Elderly” of Social Welfare Department since April 2020. We have been providing home-based and centre-based services to the elderly who are on the waiting list of Long Term Care Service. Qualified elderlies in the community can choose suitable services and receive professional and appropriate care according to their needs while remaining in a familiar community. In such way, both the elderlies and their carers can have peace of mind.

 

Service Features

 

  1. Day-care service with 3 meals provided
  2. Case management by a team of professionals with regular assessments and assistance for carers
  3. Rehabilitation treatment, cognitive training, and multisensory training
  4. Appropriate individual care together with close monitoring of health conditions and treatment progress, with referral to specialists when necessary.
  5. Physiotherapist home-based service: rehabilitation exercise training, fall prevention training, consultation on rehabilitation devices, and training carers on basic rehabilitation exercises.
  6. Occupational therapist home-based service: reality orientation and cognitive training, home safety assessment and improvement suggestions, consultation on devices and supplies for rehabilitation and elderly care, and training for carers on techniques and best practices.
  7. Home-based nurse service: health assessments, medication management, nutrition advisements, health consultation, and basic training for carers on providing care.
  8. Service details can also be arranged according to individual needs.

Who Can Apply

  1. Any elderly person on the Central Waiting List for Subsidized Long Term Care Services who has been assessed to be of moderate or severe impairment by the Social Welfare Department under the Standardized Care Need Assessment Mechanism for Elderly Services, and has not received any residential care services or subsidized community care services.
  2. A holder of Community Care Service Voucher for the Elderly.

 

Application and Withdrawal

  1. We invite anyone needing or interested in these services to contact us by phone (during office hours), email, or fax. They can also apply in person at our centre.
  2. Withdrawal can be made with an advance notice of at least 1 month.

 

Service Fees

A monthly co-payment based on Community Care Service Voucher package value which varies from $4,290 to $10,260 (rates for 2023-2024 fiscal year).

*There are 6 co-payment levels: 5%, 8%, 12%, 16%, 25%, and 40%. It is the assigned percentage used to calculate the out-of-pocket monthly co-payment based on the voucher package value chosen. The government will pay the balance.

*The assigned co-payment percentage level for each applicant is determined by the Social Welfare Department based on the family household income and financial condition of the elderly person and his/her residing family.
